The AIDS Project's HIV/AIDS Case Management Program is dedicated to helping Centre and Clinton county residents achieve optimal wellness while managing their HIV disease.
Living well with HIV/AIDS is more than going to a doctor. It requires good coordination of the following areas:
Accessing Medical Care - Finding HIV-knowlegdeable physicians, dental providers, and pharmacists.
Qualifying for Medical Insurance - Understanding private health insurance benefits, applying for and determing eligibility for public health coverage such as Medicaid, Medicare and AIDS drug assistance programs.
Meeting Basic Needs - Finding ways to cover the costs of such basic needs as housing, utilities, food, and transportation.
Treatment Adherence - Receiving information and support with achieving the high degree of medication adherence that successful HIV/AIDS drug treatment requires.
Secondary Prevention - Learning how not to infect others, how to prevent becomming infected again with other HIV strains and accessing safer sex tools.
The AIDS Project's Case Manager knows what local resources are available to HIV-positive residents AND will help coordinate those resources.
